We are available for emergencies 24
hours a day. Dial 356-9433 (our main line) and you'll reach the Doctor
on call. Our appointment hours are: Monday and Thursday: 7:30am to 6:00pm Tuesday, Wednesday and Friday: 7:30am to 5:00pm Saturday: 7:30am to 1:00pm Sunday: Closed We are located at the corner of Highway 12 (Pine Street) and Berkley Boulevard in Baraboo, Wisconsin. Please see the red star on the map below. From I-90/I-94 Take exit number 92, Highway 12 East toward Baraboo. Follow Highway 12 East approximately 6.6 miles to Berkley Boulevard on your left. Hill-Dale is on the corner as you make a left on Berkley. If you see Highways 136 and 33, you have just missed us!
